Residential Attendance - The 3 residentials of the programme are as follows; Tuesday 14th – Thursday 16th October 2025; Tuesday 10th – Thursday 12th March 2026; Tuesday 10th – Wednesday 11th June 2026 - Do you anticipate being able to attend all gatherings?(Required)

For the first residential, would you like B&B for the evening before on Monday 13th October 2025 costing £65.00? (The experience of former participants is that arriving the night before can be a helpful opportunity to switch off from work or other situation before the residential ‘proper’ begins so I would encourage you to consider this option, even if you don’t need to travel the day before)(Required)

360 Degree appraisals - These form a key part of the programme, providing helpful insights from those around you. To understand whether you have completed one before, please answer the following questions. Have you completed a 360 degree appraisal previously?(Required)

If yes, was this in you current context of ministry?

If yes, was this in the last 2 years?

If yes, are you willing to share these results and use them for Germinate Leadership?

Mentors - These will meet with you on a monthly basis, usually in person, and for around an hour each time. They should therefore be fairly local to you. They would likely have a senior position within the church structures, and may also be a different denomination to you. Do you have in mind someone who could act as Mentor for you during this programme? They should be someone who does not know you very well. If yes, please give their name and contact details below. (If not, we will work with you to identify a good mentor for you).

Work Shadowing - Offers insight into the leadership approaches of others, and helps to provide a useful contrast from your own leadership approach. Do you have someone in mind that you would wish to shadow for up to a week during the early stages of the programme. This person should be in a senior position and preferably external to the church. If Yes – Please provide their name and contact details. (If not, we will work with you to identify an appropriate placement)

Photography and Videography - On occasion, we will be taking photo's and recording video's during the programme for future promotion, with funders etc. Do you consent to us using your images in this way? Should there be any extended videos - specific permissions will be sought from you.(Required)

We would be grateful if you could upload a photograph of yourself for our reference and to share in advance with the group that you are joining (as creatively as you choose, so long as you are recognisable). (jpg or png files only please)(Required)

In further anticipation of our time together through Germinate Leadership, we would appreciate it if you could spend some time writing a profile / bio on yourself to share with those with whom you will be journeying on the programme. Do select the content of this as you see fit, however it’s less of a CV, and more of an introduction… Please do also include an indication of your current home area. We would suggest approx 800 words (equivalent to a side or so of A4). You may wish to write it elsewhere and then paste it in here once completed.(Required)
