Germinate Leadership 360 Degree Appraisals
Thank you for agreeing to complete a 360 Degree appraisal for a delegate participating in our Germinate Leadership Programme. We are excited to have them on board for this 15-month journey and truly value your input.
Your Role
Your feedback plays a vital part in helping the delegate reflect on their current leadership style and abilities, as perceived by colleagues and church members. We rely on honest and constructive input to provide meaningful insights.
By gathering feedback from eight individuals, we generate a comprehensive 360 Degree report for the delegate to review with their programme mentor. Your responses will be averaged across all participants to ensure confidentiality, allowing you to share your reflections freely. This process helps to highlight trends and patterns in their leadership, providing a solid foundation for thoughtful reflection.
Areas of Focus
The appraisal covers four key areas:
- Pathfinding: Exploring their vision and sense of purpose.
- Empowering: Evaluating their ability to release the talents and energy of others for collaborative work.
- Aligning: Considering how effectively they create a coherent system for ministry and mission.
- Modelling: Reflecting on the integrity of their personal and professional life.

When completing the form, please set aside enough time to finish in one sitting, as partial responses cannot be saved. The appraisal includes 40 questions, each asking you to assess both how well the delegate performs a task and how important it is. At the end, you’ll also be asked to identify their three greatest strengths and suggest three areas for improvement.
